MT makes small camping and snowmobile trailers. The demand for camping trailers occurs between January and June (mostly in April and May). MT makes camping trailers from January to June, shuts down in July and then makes snowmobile trailers from August to November. Suppose now is the end of December. For simplicity, we consider every two months as a period. The forecasts for camping trailers during each of the next three periods (six months) are:

MT employs 40 permanent workers who are paid an average of $20 per hour (including fringe benefits) and work approximately 320 hours a period (2 months). They make approximately 1,000 camping trailers per period during regular time. They can also work up to 50 percent more as overtime (i.e., up to 12 hours a day vs. the regular 8 hours a day) and will be paid 1.5 times the regular wage rate. Alternatively, MT can hire up to 40 additional temporary workers to work during a second shift. Hiring cost is $3,000 per temporary worker. Assume temporary workers’ wage rate and productivity are the same as permanent workers. Also assume that temporary workers work only during regular time (no overtime) and are kept for whole periods (i.e., for 2 months or 4 months). Inventory holding cost per camping trailer per period is $180 and is charged to average inventory level during each period. Currently there are no camping trailers on hand, and the desired inventory at the end of period 3 is zero (although a small positive number is also acceptable). MT wishes to meet the total demand, but shortage during a period (except last) is acceptable, in which case the shortage is assumed to be backordered at the cost of $600 per camping trailer per period.

Patient care hinges in part on adequate and timely information exchange between treating providers. Referral and reply letters are common means by which doctors and nurse practitioners exchange information pertinent to patient care. Ensuring that letters meet the needs of letter recipients saves time for clinicians and patients, reduces unnecessary repetition of diagnostic investigations, and helps to avoid patient dissatisfaction and loss of confidence in medical practitioners.

As a Nurse Practitioner (NP) you will need to know the difference between a consultation and a referral for treatment, when ordering and when carrying out consultations or referrals.

Consultations

A consultation is a request for opinion or advice, so that the requestor can manage the patient. A consultation is billed under one of the consultation codes listed in Physicians’ Current Procedural Terminology (CPT) (99241-99245 for outpatient of office consultations). If the NP is the consultant, the NP should document the request for a consultation, the reason for the consult, and the NP’s evaluation and recommendations.

When an NP requests a consultation from another provider, the N P should request “consultation” on the referral form, rather than “referring.”

Referrals:

A referral is made when the referring provider wants to turn the management of the patient over to the referred-to provider, at least for the current complaint.

When a NP refers a patient, the NP should state on the referral form that the NP is “referring the patient for evaluation and treatment.” The referred-to provider will bill an evaluation and management code, rather than a consultation code.

The following is my project submission for this project and some information may be useful for this project—

The behavior I have chosen is that of my exercise regime. Currently, I work full-time, homeschool my daughter, and attend online schooling. Due to this, I do not incorporate a lot of physical activity into my daily schedule and considering baseline data this shows my level of exercise is almost nonexistent. By choosing adding exercise to my daily routine as my topic for the self-management project I hope to become healthier. To begin this project, I will need to evaluate my bodies current abilities in regard to exercise and stamina. Determining the best approach at exercise in the amount of time allotted is imperative as well.  For each day of the project a record of which exercises completed, in addition to the amount of time dedicated will be logged. By keeping track of the different exercises and time, I should be able to modify by exercise regime to one that benefits my body and schedule, revising if deemed necessary. By keeping an observable and measurable record, I will be forced to hold myself accountable.
