You need to review and revise your Five Whys Worksheet. The items are not optimally developed. Therefore, they provide unclear information about the issue and its root causes. Taking the time to really drilling down will help you have a better understanding of the issue and, consequently, develop an effective W/CBT. The issue with your assignment is that your Five Whys were not fully developed. Therefore, it is unclear to the reader what exactly the issues really are. Fully developing each why will help you understand the root causes fully.
